Position Summary
As Remote Intake Specialist at Ready2Nurse, you cover the front of our patient workflow on off-hours (Saturdays, Sundays, and evenings). Hospitals discharge patients seven days a week; you make sure referrals that arrive on the weekends or evenings get a fast, professional response, move toward Acceptance, and land cleanly with the weekday team on Monday. You also support our clinicians remotely during their weekend visits, assembling referral packets, coordinating starts of care, handling scheduling changes. Since we are a small company, you will also support other roles and assist as needed in all areas of patient care coordination. This is a remote, part-time or full-time role worked on a California daytime schedule.
Qualifications:
Required
• Fluent, excellent spoken and written English; this is a hard requirement. You will be on the phone with US patients, families, hospital discharge planners, and physician offices, and you must represent us flawlessly
• At least 3+ year’s experience supporting US Home Health agency, specifically with intake
• Warm, persuasive phone presence: able to win the confidence of patients and referral sources in the first call and build relationships that last
• Hands-on insurance eligibility and prior authorization experience: running Medicare eligibility, checking commercial coverage through Availity or payer portals, recognizing when authorization required, and initiating and tracking auth requests
• Excellent organization skills - ability to track and manage multiple patients at various stages of intake, and work fast under deadlines.
• Able to work Saturday and Sunday on a California daytime schedule, reliably and independently
Preferred
• Proficient with Kinsser/Wellsky EHR
• Clinical degree or background (RN, PT, LVN, etc)
• Familiarity with hospital referral portals (AIDA, Careport)
• OASIS Expertise and knowledge of Home Health regulations
Job Duties:
Referral Intake
• You will primarily be responsible for “winning” patients who have contacted us initially – getting patients from initial phone call, fax, or email, through intake “acceptance” and start of care.
• Continuously monitor and respond to our various intake channels – hospital portals, fax, email & phone inquiries – and respond within 1 hour.
• Screen referrals for service area, clinical fit, and clinician capacity, and verify insurance eligibility (Medicare and Commercial) using Availity or other online portals; escalate questionable cases to leadership.
• Follow up with hospitals and physician offices for required documentation (face-to-face notes, home health orders, etc.).
• Complete outbound calls to win new referrals and develop referral relationships.
Care Coordination
• Coordinate weekend starts of care with on-call clinicians and patients: complete referral packets, complete pre-charting, order wound care supplies, etc.
• Manage weekend inbound calls from existing patients, regarding cancellations, schedule changes, and other issues; escalate to Administrator when needed.
• Assist with any other areas of patient care coordination when needed, such as QA, Billing, Schedule Management, etc.
Compensation & Schedule
• Compensation: ₱435 to ₱625/hr ($7-10/hr) depending on qualifications
• Schedule: Part-time or Full-time. 16-40 hours per week, with availability on Saturday and Sunday during CA business hours.
