Position Title: RN Generalist
Reports To: Program Manager
Location: Remote
Position Summary
The RN Generalist supports participants enrolled in Adult Day Health by assessing needs, developing individualized service plans, coordinating interdisciplinary care, and ensuring high quality, person centered support. This role maintains continuity of care through effective communication, timely documentation, and active collaboration with caregivers, clinicians, and internal tea
Essential Functions
Client Assessment & Care Planning
• Determines clients’ needs by establishing rapport with participants and caregivers, identifying goals, and coordinating interdisciplinary assessments.
• Develops and implements individualized care plans for clients with complex needs and chronic conditions. Includes both initial care plans and ongoing care plans quarterly.
• Adjusts services and care plans based on ongoing client evaluation and progress toward goals.
Care Coordination & Communication
• Maintains continuity of care by documenting and communicating actions and updates in accordance with WAC requirements and Full Life Care policies.
• Communicates changes in client condition to internal tea
• Participates in telehealth services, including virtual visits and phone communication with clients and caregivers.
• Requests medical reports and orders for new enrollees and annually thereafter.
• Requests annual care plan signature from client/guardian.
• Sends updated care plans to client’s DSHS or VA case manager.
• Follows up with new enrollees to plan start of care at Adult Day Health.
Team Collaboration
• Works closely with the nurses, social worker, intake specialist, program manager and director to coordinate client services.
• Participates in interdisciplinary meetings and contributes to care planning, problem solving, and client advocacy.
Documentation & Compliance
• Maintains accurate, complete, and timely case management records in compliance with agency policies and Washington Administrative Code (WAC) requirements.
• Completes required attendance documentation and inputs monthly data into Cadcare.
• Reconciles client’s medication in Cadcare.
Administrative Support
• Answers incoming calls, provides information to caregivers and providers, and schedules site visits.
• Manages required documentation and updates to ensure regulatory compliance and operational efficiency.
• Assists with submitting Access transportation applications and follow through
• Complete projects and tasks assigned by supervisor;
Qualifications
• RN licensure
• Experience working with older adults or individuals with disabilities strongly preferred.
• Strong communication, organization, and documentation skills.
• Ability to work effectively within a multidisciplinary team.
• Proficiency in electronic record systems; Cadcare experience a plus.
