We are seeking a dedicated and experienced Remote RN Case Manager to join our dynamic team. In this pivotal role, you will ensure the highest level of compliance and quality care for our clients. Your responsibilities will include overseeing clinical documentation, managing patient care updates, and supporting the onboarding process for new clients. You will work closely with our per diem RNs and clients to ensure continuity of care and optimal outcomes. As an integral part of our team, you will also play a key role in the recruitment and training of nurses, all while maintaining excellent communication and coordination with our clients.
What You Will Do:
As a member of the Clinical team at Alvita Care, the Remote RN Case Manager is expected to support Operations in the provision of home care for our clients.
• Ensure form compliance with IA and POC updates, and on RN hourly and SNV visits
• Join New Jersey and Long Island daily market meetings
• Conduct the clinical portion of the caregiver weekly orientation
• Join welcome calls for new clients
• Manage clinical portion of all new SOCs for the first 14 days
• Work with hospitals and sales to obtain any clinical/discharge paperwork
• Conduct clinical consultations for pre-SOC questions, especially for potential nursing clients
• Manage and train per diem RNs doing IA and POC updates
• Follow-up with per-dem nurses after client visits
• Assist in onboarding and training per diem nurses
• Conduct NJ POC phone calls every 60 days for all clients
• Provide oversight of care and communication to selected clients
• Clinical admin work as needed (e.g., form approvals)
What You Will Need:
• Registered nurse (RN) with active, unencumbered licensure to practice professional nursing in the states of New Jersey and New York
• 4+ years of experience in acute care setting required
• Experience in home health, home care and/or hospice required
• A bachelor’s degree in nursing is preferred, but not required
