We are seeking a detail-oriented and compassionate Medical Intake Coordinator to manage patient intake processes for our Home Health service in California. This role serves as the primary point of contact for new referrals, patients, families, and referral partners, while also supporting light marketing and relationship-building efforts with existing referral sources.
Key Responsibilities:
1. Receive and process new hospice and home health referrals via phone, fax,
2. Verify patient demographics, insurance eligibility, and medical documentation in compliance with HIPAA, Medicare, and California state regulations.
3. Coordinate and schedule initial assessments, admissions, and start-of-care visits with clinical staff.
4. Communicate clearly and compassionately with patients, families, physicians, hospitals, and referral partners.
5. Maintain accurate, complete, and confidential patient records in accordance with HIPAA privacy and security standards.
6. Track referral status, follow-ups, and outcomes to ensure timely admission or appropriate disposition.
7. Collaborate with clinical, billing, and administrative teams to ensure smooth transitions from intake to care delivery.
8. Provide updates to existing referral sources regarding patient status and admission outcomes.
9. Support minimal marketing activities such as maintaining referral lists, sending updates or materials to existing referral partners, and assisting with referral relationship management.
10. Assist with intake and referral reports as needed.
Requirements:
Previous experience in medical intake, hospice, home health, or healthcare coordination preferred.
Working knowledge of HIPAA compliance, patient confidentiality, and proper handling of PHI.
Strong verbal and written communication skills in English.
Excellent organizational skills and attention to detail.
Ability to handle sensitive situations with professionalism, discretion, and empathy.
Proficiency with EMR systems and Microsoft Office or similar platforms.
Ability to multitask and work effectively in a fast-paced healthcare environment.
Preferred Qualifications:
Experience working with US-based hospice or home health agencies.
Familiarity with insurance verification, authorizations, physician orders, and Medicare guidelines.
Formal training or hands-on experience in HIPAA compliance and healthcare documentation standards.
Background in intake coordination, customer service, or referral management.
Monthly Rate: PHP 20,000
Work Schedule: Monday to Friday, 8:00 AM – 5:00 PM PST
