We are seeking an experienced, proactive Home Health Coordinator to play a key role in managing daily operations and ensuring seamless service delivery.
Position Summary
The Home Health Coordinator is responsible for overseeing the coordination of client services, staff scheduling, and administrative workflows. This role manages client intakes, ensures compliance, supports recruitment efforts, and leads a team of Virtual Assistants (VAs) who assist with intake, documentation, and scheduling tasks.
This is a senior coordination role requiring prior experience in home health operations, strong leadership skills, and the ability to manage multiple priorities in a fast-paced, remote environment.
Key Responsibilities
Manage the full client intake process, including collecting, reviewing, and verifying required medical documentation
Communicate professionally and effectively with families, caregivers, nurses, Virtual Assistants, and agency leadership
Coordinate nurse and caregiver schedules to ensure full and consistent shift coverage
Track and maintain compliance documentation, certifications, supervisory visits, and related records
Collaborate with RNs and leadership to ensure appropriate care plans and service delivery
Lead, supervise, and provide ongoing guidance to a team of Virtual Assistants supporting administrative and coordination functions
Support recruitment efforts by screening applicants, scheduling interviews, and assisting with onboarding
Proactively identify and resolve staffing or scheduling issues
Complete administrative tasks such as faxing,
Qualifications
Minimum of 2–3 years of experience as a Home Health Coordinator or in a similar role within home health or healthcare staffing
Demonstrated leadership experience managing, mentoring, or overseeing a team
Excellent written and verbal communication skills
Strong organizational skills with the ability to multitask and meet deadlines
Proficiency with scheduling platforms, cloud-based tools, and electronic documentation systems
Ability to work independently in a remote environment while collaborating in real time
Experience with pediatric home health, medically fragile populations, or caregiver staffing strongly preferred
