HOME HEALTH CREDENTIALed CODER & QA SPECIALIST

Remote | U.S. Medicare Home Health | Per-Chart Compensation

Wellspring Home Health Center, LLC is seeking an experienced Home Health Credentialed Coder / QA Specialist to support our Medicare home health clinical documentation, coding, and quality review.

This position will begin on a part-time, per-chart basis and has the opportunity to grow in hours, responsibilities, and compensation as Wellspring's Medicare census increases.

This is NOT a general Virtual Assistant or general medical coding position. Applicants must have direct experience working with a U.S. Medicare-certified home health agency.

Compensation

Starting at $13 per completed chart/review

Rate may increase based on:

Coding and OASIS experience

HCS-D/BCHH-C and additional credentials

Accuracy and quality of work

Ability to independently complete coding and QA reviews

Turnaround time

Documentation and compliance knowledge

Demonstrated performance

Increasing Medicare census and workload

As Wellspring's Medicare census grows, this position may expand into a larger part-time or full-time Coding/QA role with increased compensation.

Required Credentials

Candidate must have at least one of the following:

HCS-D – Home Care Coding Specialist-Diagnosis

BCHH-C – Board Certified Home Health-Coding

Strongly Preferred:

COS-C – Certificate for OASIS Specialist-Clinical

HCS-O – Home Care Clinical Specialist-OASIS

Candidates with both home health coding and OASIS credentials are strongly preferred.

Required Experience

Direct experience with a U.S. Medicare-certified home health agency

Strong ICD-10-CM coding and sequencing knowledge

Strong understanding of PDGM

Experience reviewing OASIS

Experience reviewing Face-to-Face (F2F) documentation

Experience reviewing Plan of Care / CMS-485

Understanding of Medicare home health eligibility and documentation requirements

Ability to identify coding, documentation, and compliance problems before billing

Experience communicating coding/documentation queries to clinicians when clarification is needed

Understanding of primary and secondary diagnosis selection and sequencing

Ability to ensure diagnoses are supported by the medical record

Knowledge of Medicare home health documentation and reimbursement requirements

Primary Responsibilities

The Coder/QA Specialist will review assigned charts for:

ICD-10-CM coding accuracy and sequencing

Appropriate primary and secondary diagnoses

PDGM clinical grouping

OASIS consistency and accuracy

Diagnosis/OASIS consistency

Face-to-Face documentation

Physician/allowed practitioner orders

Plan of Care / CMS-485

Skilled need and homebound documentation

Medication and diagnosis consistency

Hospital/SNF referral documentation

Start-of-care documentation

Clinical documentation completeness

Potential billing or compliance concerns

Missing, conflicting, or unsupported documentation

The specialist will provide a clear QA/coding report identifying corrections, clarification needs, and recommendations before the chart proceeds to billing.

Quality Expectations

We are looking for someone who can do more than simply assign diagnosis codes.

The successful candidate must be able to look at the entire home health record, identify inconsistencies, and communicate clearly what needs to be corrected or clarified.

Accuracy and Medicare compliance are more important than speed.

Coding must always be supported by the medical record. The coder should never select or change a diagnosis solely to increase reimbursement.

Turnaround Expectations

Assigned charts should generally be reviewed within the agreed turnaround time.

Urgent SOC/admission charts may require priority review.

Questions or missing documentation should be reported promptly.

The coder must maintain an organized record of charts received, completed, returned for clarification, and finalized.

Remote Work Requirements

Candidate must have:

Reliable computer capable of handling multiple healthcare systems

Reliable high-speed internet

Backup internet plan

Private, HIPAA-appropriate workspace

Strong computer skills

Secure handling of PHI

Ability to communicate during agreed U.S. business hours when necessary

Growth Opportunity

This position is being established as Wellspring continues to grow its Medicare home health census.

The successful candidate may begin with a smaller number of charts on a per-chart basis.

As census and chart volume increase, Wellspring may increase:

Number of charts assigned

QA responsibilities

Coding responsibilities

Weekly hours

Compensation

Opportunity for a regular part-time or full-time position
