# Remote Experienced Cardiothoracic Medical Coder / Biller
A busy cardiothoracic surgical group is seeking an experienced **Medical Coder/Biller with strong cardiothoracic surgery experience** to join its fully remote team. The ideal candidate will have advanced knowledge of surgical coding and billing workflows, with a focus on cardiothoracic, cardiovascular, and related complex surgical procedures.
## Position Details
* **Work arrangement:** Fully remote
* **Schedule:** Full-time, 40 hours per week
* **Compensation:** $5 per hour
* **Experience level:** Experienced medical coder/biller
* **Preferred background:** Cardiothoracic surgery, cardiology, vascular surgery, or other high-acuity surgical specialties
## Responsibilities
* Independently review operative reports, progress notes, and supporting clinical documentation to assign accurate CPT, ICD-10-CM, and HCPCS codes
* Apply advanced cardiothoracic coding knowledge to interpret complex surgical procedures, including open-heart, thoracic, and vascular interventions
* Determine appropriate professional billing components, including global surgical packages and separately reportable services
* Identify and apply correct modifiers, including assistant surgeon, co-surgeon, staged or related procedures, repeat procedures, and multiple procedure scenarios
* Ensure compliance with NCCI edits, global surgery rules, and payer-specific cardiothoracic billing guidelines
* Query surgeons or clinical staff when documentation is insufficient or unclear for accurate coding
* Prepare and submit clean inpatient and outpatient claims through electronic billing systems
* Perform detailed claim scrubbing to verify CPT/ICD accuracy, modifier usage, POS, and insurance eligibility
* Post electronic remittance advice (ERA) and explanation of benefits (EOB) accurately and reconcile payments
* Manage contractual adjustments, patient balances, and insurance balances with precision
* Enroll and maintain EFT and ERA setups with commercial and government payers
* Monitor claim status and proactively follow up on unpaid, denied, rejected, or underpaid claims
* Analyze clearinghouse and payer rejections and correct claims for resubmission
* Investigate and resolve insurance denials, including cardiothoracic-specific coding and medical necessity issues
* Prepare and submit corrected claims, reconsiderations, and formal appeals as needed
* Work aging reports and prioritize claims approaching timely filing limits
* Identify recurring denial trends and communicate findings to management for process improvement
* Maintain thorough and accurate account documentation for all billing and follow-up activity
* Support additional billing, coding, and administrative functions as assigned
## Qualifications
* Proven experience as a medical coder and biller in a surgical specialty is required
* Strong, hands-on experience coding cardiothoracic, cardiovascular, or complex surgical procedures is strongly preferred
* Demonstrated ability to accurately interpret operative reports and assign CPT/ICD-10-CM codes independently
* Solid understanding of inpatient and outpatient billing workflows
* Experience with Medicare, Medicaid, and commercial insurance billing requirements
* Advanced knowledge of CPT, ICD-10-CM, HCPCS, modifiers, NCCI edits, and global surgical rules
* Familiarity with assistant surgeon, co-surgeon, staged, repeat, and multiple procedure billing scenarios
* Experience submitting and managing claims through a practice management system or clearinghouse
* Strong understanding of ERA/EOB posting, payment reconciliation, and account balancing
* Experience with EFT/ERA enrollment and payer credentialing processes
* Strong denial management, appeals, and insurance follow-up experience
* Ability to interpret payer correspondence, denial codes, and remittance advice accurately
* Coding certification (CPC, CCS, or equivalent) preferred
* High attention to detail with strong accuracy in coding and billing workflows
* Excellent written and verbal communication skills in English
* Ability to work independently in a high-volume, deadline-driven environment
* Reliable computer, high-speed internet, and secure quiet workspace
* Strict adherence to HIPAA and patient confidentiality requirements
