In this role, you will provide person-directed, individualized support to assist clients with psychiatric disabilities in restoring their community functioning and well-being.
Direct Client Care: Implement individualized interventions from the client’s treatment plan, teaching and modeling positive coping and social skills.
Life Skills Coaching: Guide clients through independent living skills, Activities of Daily Living (ADLs) support, and practical milestones like obtaining vital credentials (IDs, social security cards, etc.).
SKILL REQUIREMENT
Clinical Collaboration: Work closely with the Rehabilitation Coordinator and treatment teams to plan interventions for complex cases and track client progress.
Crisis Prevention: Actively participate in behavioral de-escalation, crisis intervention, and proper documentation. Rigorous Documentation: Maintain accurate, timely electronic medical records (EMR), ensuring all daily progress notes are logged within 24 hours.
Client-Centered Communication: Strong interpersonal skills tailored for mental health environments. Team Collaboration: Ability to work with treatment teams and clinical supervisors.
Bilingualism: Multi-lingual communication skills (preferred).
Technical Proficiency: Skilled in utilizing MS Office (Word, Excel), RingCentral (for client communication/telehealth), and Dropbox (for secure document storage and file sharing).
Reporting & Data Tracking: Ability to compile, organize, and generate detailed administrative reports tracking client outreach and daily contact logs.
Maintain open lines of communication with clients utilizing platforms like RingCentral. Securely manage, upload, and organize client documentation and collaboration forms using Dropbox. Utilize MS Office to track outreach data and generate accurate reports regarding the number of clients contacted and services provided.
