HIRING: Care Coordinator – CCM Program

Full-Time Remote Virtual Assistant | Immediate Hire

? Philippines-Based Applicants | 100% Remote

We are looking for a reliable, compassionate, and highly organized Care Coordinator to support our Chronic Care Management (CCM) program. This role is primarily call-based and involves patient communication, EHR documentation, follow-ups, and coordinating non-clinical patient needs.

Responsibilities
• Cold call patients from EHR list and do monthly care calls
• Document patient interactions accurately in the EHR.
• Coordinate appointments, referrals, medication-related follow-ups, transportation, and community resources.
• Track outstanding patient needs and follow through appropriately.
• Escalate concerns to the appropriate clinical team member.
• Maintain patient confidentiality and follow HIPAA requirements.

Qualifications
REQUIRED:
• Experience in at least one of the following: Healthcare call center/customer service; OR bedside nursing or direct patient care.
• Strong written English communication and documentation skills.
• Reliable internet connection and a suitable work-from-home setup.
• Organized, dependable, compassionate, and willing to learn.
• Able to work full-time and maintain consistent attendance.
A nursing license is NOT required. Healthcare, medical VA, care coordination, CCM, and patient-support experience are preferred but not mandatory.
• Previous experience using an Electronic Health Record (EHR/EMR) system is a plus but not required

Training Commitment
This position includes a structured six-week training and onboarding program. Full attendance throughout training is essential. Applicants must be able to commit to the entire training period without planned vacations or avoidable absences.
