About Us: United Rehab Group (URG) provides home-based physical and occupational therapy services across Southern California. Our mission is to help patients recover and improve their quality of life in the comfort of their homes — with care, precision, and compassion.

What We’re Looking For: Discharge Coordinator
We are looking for a highly organized and detail-oriented Care Coordinator to support our growing home health operations.

* Key Responsibilities:
- Distribute patient caseloads and assign them to therapists
- Plot and update schedules in the system accurately
- Quality-check all patient charts pending discharge
- Send and track Discharge Requests, ensuring timely completion
- Coordinate and report with insurance and handle other related administrative tasks

* Compensation: $225/month (fixed)
* Working Schedule: Sunday - Friday starting 1pm - 5pm (Pacific Time)

Qualifications:
- BPO experience or remote work experience is highly desired
- No other work commitments preferred
- Healthcare or home health experience is a significant advantage
- Excellent ability to multitask and perform well in high-pressure settings
- Stable wired internet connection (minimum 20 Mbps download & upload)
- Reliable device (minimum 8GB RAM)
- Quiet, dedicated workspace and noise-canceling headset
- Backup power and internet connection available
