Job Overview:
The Care Case Manager is a licensed nursing professional who supports the delivery of high-quality home care services by reviewing caregiver documentation, care notes, monitoring client conditions, and providing clinical guidance to families. This role serves as a key liaison between caregivers, clients, and families, ensuring care is accurate in volume and aligned with client needs in a home care environment.
In addition, this role operates in compliance with HIPAA regulations and the organization’s Business Associate Agreement (BAA), ensuring the secure and appropriate handling of Protected Health Information (PHI).
Key Responsibilities:
- Review caregiver care notes and documentation to ensure accuracy, completeness, and quality of care provided in the home setting
- Monitor and respond to critical alerts or changes in client condition, escalating concerns as appropriate
- Provide clinical analysis and recommendations based on care trends and client status
- Communicate regularly with clients and families to provide updates, education, and guidance on care needs
- Conduct routine wellness checks (via phone/email)
- Collaborate with scheduling and care coordination teams to adjust services based on client needs
- Support caregivers by providing guidance and reinforcing best practices in home care delivery
- Ensure compliance with company policies, state regulations, and home care standards
- Maintain timely and accurate documentation of all interactions and clinical observations
HIPAA & Data Privacy Responsibilities:
- Adhere strictly to the Business Associate Agreement (BAA) between Amada Senior Care of East Bay and all workforce members
- Use and disclose Protected Health Information (PHI) only as necessary to perform assigned duties and in accordance with HIPAA (45 CFR Parts 160 and 164)
- Ensure PHI is accessed, used, and shared only within the minimum necessary scope
- Maintain confidentiality of all client information and avoid unauthorized use, disclosure, or distribution of PHI
- Immediately report any suspected or confirmed data breach or security i
- Utilize only HIPAA-compliant systems, platforms, and communication tools
Safeguards & Compliance:
- Follow administrative safeguards including role-based access and confidentiality agreements
- Maintain secure login practices (no shared accounts) and use multi-factor authentication where applicable
- Ensure all documentation and communication systems are encrypted and compliant
- Maintain a secure and private work environment to prevent unauthorized exposure of PHI
- Comply with offshore workforce standards (if applicable), ensuring PHI is accessed only through secure systems and not stored locally without approval
- Support audits, reporting, and documentation requirements related to PHI access and disclosures
Qualifications:
- Active and valid nursing license (RN required)
- Experience in home care, hospice, or case management preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication skills with ability to support families remotely and in person
- Ability to work independently in a fast-paced, field-based environment
- Proficiency in electronic documentation systems
