Please read this posting in full before submitting.

Codes Health is a well-funded, fast-growing US startup building the modern platform for medical records retrieval. We work with law firms and specialty healthcare providers to get records faster, with zero errors.

BEFORE YOU APPLY
As the first step in our process, we ask all applicants to send a short video recording (2–3 minutes max) answering the following three prompts:

1. Tell us about yourself — what kind of work you've done before, and why you're interested in this role.
2. Tell us about a mistake you made at work. What happened and what did you do about it?
3. Tell us about a time you improved a process at work. What did you change, and what was the result?

Applications without a video answering the 3 questions will not be reviewed.

ABOUT THE ROLE
Our provider database is the foundation every records request is built on: facility names, records department contacts, fax numbers, submission methods, and billing-company details. Wrong information is worse than no information, because a bad fax number doesn't surface until it has already delayed a live case. This role owns the quality of that database. You'll start by auditing and cleaning what's already there, then move into re-validating providers on a schedule so the data stays trustworthy as we grow.

WHAT YOU'LL DO
• Audit the existing provider database entry by entry: fix formatting, standardize names and addresses, merge duplicates, and flag fields that are missing or look wrong
• Apply and enforce data standards so every provider record is complete and consistently formatted
• Cross-check entries against provider websites, the NPI registry, and records portals (Datavant, ChartSwap, MRO, and others)
• Re-validate previously verified providers: call records departments to confirm fax, phone, submission method, and third-party billers are still correct, and update the record with a verified timestamp
• Spot-check the validation team's recent work for accuracy and report error patterns back to the team lead
• Review output from AI-assisted lookups against known-good providers and flag where it is wrong
• Track your audit progress and surface data-quality metrics (entries reviewed, errors found, fields corrected)
• Suggest ways to catch bad data earlier and keep the database clean with less manual effort

WHAT SUCCESS LOOKS LIKE
• The provider database is clean, consistently formatted, and the validation team trusts it enough to stop using side spreadsheets
• Requests rejected or misrouted because of stale provider data drop toward zero
• Every provider you touch has a verified timestamp and a clear record of what was confirmed and how
• Error patterns you find turn into fixes in the validation process, not just fixes to individual records

WHAT WE'RE LOOKING FOR
• Strong English proficiency — written and spoken (required)
• Meticulous and systematic — you enjoy working through a large dataset carefully, one record at a time, and you don't guess
• Strong data hygiene instincts — you notice inconsistent formatting, duplicates, and fields that don't add up
• Comfortable on the phone — able to call US provider offices to confirm details and navigate phone trees
• Resourceful researcher — you find the right department and contact even when the information isn't obvious
• Organized and reliable — able to track progress across hundreds of records without losing your place
• Quick to learn new tools and software workflows (admin dashboards, spreadsheets, portals)
• Ownership mindset — you follow through until the job is done
• Prior experience in data entry, data QA, healthcare admin, medical records, or provider data management is a plus but not required

WHY CODES HEALTH - Our Website: ----------
• YC-backed, well-funded, generating revenue and growing fast
• Remote, full-time role
• High-impact position with real visibility into a growing US company
