The Spanish speaking Case Manager for Psychiatric Services is responsible for coordinating comprehensive mental health care for clients with psychiatric disorders. The role involves assessing client needs, developing individualized service plans, linking clients to appropriate community resources, and ensuring continuity of care to promote recovery and stability.

Key Responsibilities:
Conduct comprehensive psychosocial assessments to determine client needs and eligibility for services.
Develop, implement, and monitor individualized care plans in collaboration with clients, families, and multidisciplinary teams.
Coordinate psychiatric and therapeutic services, including medication management, counseling, and crisis intervention.
Link clients with community resources such as housing, employment support, benefits, and social services.
Advocate for client rights and needs within the mental health and social service systems.
Maintain accurate, timely, and confidential documentation in compliance with organizational and regulatory standards.
Monitor client progress and revise care plans as needed to reflect changes in condition or circumstance.
Participate in case conferences, clinical supervision, and team meetings.
Assist with discharge planning and transition of care when necessary.
Respond to crises as needed, including coordinating emergency services and safety planning.
Qualifications:
Education: Bachelor’s degree in Social Work, Psychology, Nursing, or related field required; Master’s degree preferred.
Experience: Minimum of 1–2 years in case management or psychiatric/behavioral health setting.
