We are hiring a compassionate and resourceful Remote Virtual Care Coordinator to assist patients with accessing healthcare and essential community services. This position focuses on Social Determinants of Health, resource navigation, client follow-up, and communication with community-based organizations.
This is an excellent opportunity for someone with experience in care coordination, case management support, social services, customer service, patient navigation, or detailed online resource research.
Schedule
Full-time remote position
Must be available during assigned United States business hours
Candidates must be comfortable working an overnight schedule when required
Exact working hours will be discussed during the interview
Must be available to start immediately
Key Responsibilities
Conduct patient intake calls and basic needs assessments
Identify patient needs involving housing, food, transportation, utilities, employment, behavioral health, benefits, medication affordability, and financial assistance
Research appropriate healthcare and social-service programs
Verify program contact information, eligibility requirements, service areas, and application procedures
Match patients with resources based on their needs, eligibility, insurance, and geographic location
Contact community organizations and service providers on behalf of patients when appropriate
Submit referrals and assist with completing intake or enrollment steps
Follow up with patients and organizations to determine whether services were received
Document all calls, referrals, outcomes, barriers, and follow-up plans clearly
Maintain accurate patient records in the assigned care-management platform
Communicate with patients in a warm, respectful, and nonjudgmental manner
Advocate for patients who are experiencing barriers to receiving services
Provide status updates to the care team and escalate urgent concerns appropriately
Build and maintain knowledge of community programs throughout the United States
Follow established workflows, documentation standards, and privacy requirements
Required Qualifications
Experience in care coordination, case management support, social services, healthcare navigation, community outreach, or a related role
Strong spoken and written English skills
Excellent telephone communication and active-listening abilities
Ability to speak compassionately with vulnerable or distressed individuals
Strong online research and critical-thinking skills
Ability to locate resources beyond general referral services such as 211
Excellent organization, follow-up, and time-management skills
Ability to manage several cases and priorities independently
Accurate and professional documentation skills
Dependable attendance and punctuality
Ability to follow standard operating procedures while using good judgment
Must be available to start immediately
Preferred Qualifications
Experience researching United States community resources
Knowledge of Social Determinants of Health
Experience with housing, transportation, food, utility, behavioral health, disability, or financial-assistance programs
Experience working with Medicare, Medicaid, or commercial insurance populations
Familiarity with care-management platforms, CRM systems, or electronic health records
Previous experience communicating with nonprofit organizations, healthcare providers, and government agencies
HIPAA training or experience handling confidential patient information
Technical Requirements
Reliable computer in good working condition
Stable high-speed internet connection
Backup internet connection or reliable contingency plan
Quality noise-canceling headset
Quiet and private workspace
Reliable electricity with a backup plan strongly preferred
Ability to participate in video meetings, training sessions, and telephone calls
Compensation
$3.75–$4.50 USD per hour
Final rate will be determined based on experience, communication skills, research ability, and overall qualifications

We need someone who is compassionate but also persistent. The ideal candidate does not simply send patients a list of telephone numbers. You should be willing to research thoroughly, verify information, contact organizations, complete follow-ups, and help patients understand their next steps.
You must be reliable, proactive, comfortable working independently, and able to communicate professionally with patients, caregivers, healthcare teams, and community organizations.
