We are a healthcare consulting firm based in the US serving skilled nursing facilities. We move fast and hold ourselves to a high standard. We are hiring a Medical Records Clerk to maintain accurate, complete, and audit-ready resident charts in our electronic health record system. This is a detail-driven role. You will monitor shared inboxes, upload documents to the correct charts, audit for completeness, and track missing items. Your Role
Monitor and review shared inboxes throughout the day.
Upload incoming faxes, scans, and documents to the correct resident chart in PCC.
Audit charts for completeness against admission, clinical, and compliance checklists.
Track missing documents, physician orders, and signatures, and follow up until resolved.
Verify face sheets, demographics, and insurance information for accuracy.
Flag discrepancies, misfiled documents, and incomplete records for correction.
Maintain document tracking logs and update status daily.
Support chart audits and prepare records for surveys, reviews, and compliance checks.
What You Will Need
Required:
Proven experience in medical records, health information management, or healthcare administration.
Strong attention to detail and accuracy in document handling.
Ability to audit charts against checklists and catch missing or incorrect items.
Comfortable working in Teams as primary communication tool.
Strong written and spoken English.
Self-managing, dependable, and organized.
Available Monday through Friday, 9:00 AM to 5:00 PM EST with no schedule conflicts.
Knowledge of HIPAA compliance and secure data handling.
Familiarity with chart audit processes and survey preparation.
Equipment Requirements
Reliable computer with at least 8 GB RAM and current operating system.
Stable wired internet of 25 Mbps or higher.
Backup internet connection, mobile data, or secondary connection.
Quality headset for Teams calls.
Quiet, dedicated workspace.
