West Orange Nephrology is a physician-owned kidney care clinic located in Winter Garden, Florida. We provide comprehensive nephrology services including chronic kidney disease management, dialysis, diagnostics, and laboratory services. We are expanding our remote team and looking for an experienced Outpatient Medical Billing and Coding Manager to take full ownership of our revenue cycle. This is a senior leadership role and we are looking for someone with real management experience, US medical office experience, and the credentials to back it up.
About the role:
RPM/CCM Coordinator is responsible for supporting patients with chronic conditions through Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. This role involves monitoring patient health data, conducting regular outreach, coordinating care, educating patients on managing their conditions, and ensuring accurate documentation for compliance and billing. The coordinator works closely with healthcare providers to identify potential health concerns, improve patient outcomes, and enhance the overall patient experience while adhering to Medicare and payer guidelines. Strong communication, organizational skills, and experience with electronic health records (EHR) are essential for success in this role.
Key Responsibilities
1. Monitor RPM patient data and identify abnormal readings.
2. Conduct monthly CCM patient outreach and follow-up calls.
3. Document all patient interactions accurately in the EHR.
4. Coordinate care with providers and clinical staff.
5. Educate patients on managing chronic conditions.
6. Track compliance with RPM/CCM program requirements.
7. Assist with billing documentation and reporting.
8. Escalate urgent patient concerns to the appropriate provider.
Who we're looking for:
1. Experience working in Remote Patient Monitoring (RPM) and/or Chronic Care Management (CCM) programs.
2. Required: Hands-on experience using the eClinicalWorks (eCW) platform.
3. Strong communication and patient engagement skills.
4. Excellent organizational skills with high attention to detail.
5. Ability to document accurately and manage multiple tasks in a fast-paced environment.
6. Comfortable working independently while collaborating with a remote healthcare team.
7. Knowledge of HIPAA and patient confidentiality standards.
