Position Overview:
We are seeking a highly organized and compassionate Service Officer to join our team. The Service Officer will be responsible for overseeing and coordinating services for our clients, ensuring they receive the best possible care and support. This role involves working closely with clients, families, and service providers to develop and implement individualized care plans.

Key Responsibilities:
* Assess clients' needs and develop personalized care plans in collaboration with clients, families, and service providers.
* Coordinate and monitor services to ensure they meet the clients' needs and goals.
* Act as a primary point of contact for clients and their families, addressing concerns and providing support as needed.
* Maintain accurate and up-to-date records of client interactions, care plans, and service delivery.
* Collaborate with other team members and external providers to ensure seamless service delivery.
* Advocate for clients' needs within the organization and with external agencies.
* Ensure compliance with all relevant regulations and standards of care.

Qualifications:
* Bachelor's degree holder.
* Previous experience in a service officer or case management role, preferably within the healthcare or social services sector.
* Strong interpersonal and communication skills, with the ability to build relationships with clients, families, and service providers.
* Proficiency in ClickUp and other relevant software.

Working requirements:
* Must have a fast PC or laptop
* Must have a reliable internet connection and a backup
* Must have a noise cancelling headset.
* Must be able to work from 6am-3pm PH time, Mondays until Fridays
