Clinical Care Coordinator (RN) – Family Engagement
Remote | Philippines-Based

About the Role
We are seeking a compassionate, proactive, and communication-focused Registered Nurse (RN) to join our growing remote care team. This role combines clinical judgment, family engagement, and care coordination to help seniors receive the right level of support at the right time.

Using AI-assisted client monitoring tools and care insights, you will review client i ---------- , identify potential care concerns, speak with families and clients, and recommend appropriate care solutions to improve safety, quality of life, and continuity of care.

This is an ideal opportunity for nurses who enjoy relationship-building, consultative conversations, problem-solving, and helping families navigate care decisions.

Key Responsibilities
- Review AI-generated client alerts, i ---------- , and care insights
- Assess potential changes in client condition or care needs
- Conduct outbound calls with clients, families, and caregivers
- Communicate care concerns clearly and compassionately
- Recommend appropriate additional care hours or support services when needed
- Help families understand risks, care gaps, and available solutions
- Document interactions and care recommendations accurately
- Collaborate with care teams to support client retention and satisfaction
- Follow workflows, KPIs, and quality standards for outreach and engagement

What We’re Looking For
- Active Registered Nurse (RN) license
- Excellent spoken and written English communication skills
- Strong phone presence and confidence speaking with families
- Empathy combined with professionalism and initiative
- Comfortable having consultative conversations and making recommendations
- Organized, detail-oriented, and tech-savvy
- Ability to work independently in a remote environment
- Experience in home care, case management, telehealth, patient engagement, intake/admissions, or care coordination is highly preferred
