JOB OVERVIEW

Role Summary
The Resource Specialist serves as the primary bridge between a client’s clinical rehabilitation plan and the practical, real-world resources they need to succeed. Reporting directly to the Rehabilitation Coordinator, this role focuses on high-touch client interaction to identify social, financial, and logistical gaps and proactively sourcing solutions to fill them.

Core Responsibilities
- Direct Client Needs Assessment: Lead intensive one-on-one consultations with clients to identify specific barriers to recovery (e.g., housing instability, lack of transportation, food insecurity, or financial strain).
- Resource Mapping & Vetting: Build and maintain a comprehensive directory of community partners, NGOs, government programs, and private charities that can provide immediate assistance.
- Case Support for the RC: Act as the "boots on the ground" for the Rehab Coordinator by managing the non-clinical logistics of a client’s recovery plan.
- Application Assistance: Guide clients through the technical process of applying for social security, disability benefits, healthcare subsidies, or vocational training grants.
- Advocacy & Liaison: Contact external agencies on behalf of the client to expedite services and ensure they are receiving the full scope of available support.
- Goal Tracking: Regularly interview clients to monitor the effectiveness of provided resources and pivot strategies if a particular resource is no longer a fit.

- Empathy & Professionalism: Ability to build rapport with diverse populations while maintaining clear professional boundaries.
- Organization: Strong multitasking skills to manage a high volume of client requests and resource tracking.
- Problem-Solving: Proactive approach to finding creative solutions for clients with limited access to traditional support systems.
