Job Summary
The Social Worker (Resource Specialist) serves as the primary point of contact for clients navigating their rehabilitation journey. In support of the Rehabilitation Coordinator, this role focuses on identifying individual client needs, sourcing community resources, and ensuring that social, financial, and logistical barriers to recovery are addressed.
Key Responsibilities
- Client Intake & Assessment: Conduct comprehensive interviews with clients to identify psychosocial needs, housing stability, financial constraints, and vocational goals.
- Resource Coordination: Research, maintain, and update a database of community resources, including government assistance programs, non-profits, support groups, and vocational training.
- Client Advocacy: Act as a liaison between the client and external agencies to ensure the delivery of necessary services (e.g., insurance claims, housing applications).
- Case Support: Assist the Rehab Coordinator in developing individualized service plans; monitor client progress and adjust resource allocation as needs evolve.
- Crisis Intervention: Provide immediate support and referral services for clients experiencing acute social or emotional distress during their rehab process.
- Documentation: Maintain accurate, confidential, and up-to-date case notes and resource logs in compliance with healthcare privacy standards.
