About the Project
We’re applying to become a certified service provider under the Illinois Department on Aging Community Care Program (CCP). This is a 3-step online process that starts with Step 1: Legal Entity Application, then Step 2: Service-Specific Application(s), and ends with Step 3: Submission Form.
We need an experienced attorney (or attorney-level compliance professional) who can guide us through the process, ensure accuracy, and help assemble required documentation so the submission is clean, complete, and not rejected.
What You’ll Do
Lead and support completion of Step 1: Legal Entity Application (DocuSign-based) and ensure all required fields and attachments are properly handled.
Advise on the required documents/attachments and help us prepare them in acceptable format.
Help select and complete Step 2: Service-Specific Application(s) (In-Home Services and/or Adult Day Services), aligned to our business model.
Quality-check the full packet to ensure Step 1 and Step 2 are both complete (applications may not be accepted if either is missing).
Provide practical compliance guidance, including how to respond to agency questions and avoid common submission errors (ex: DocuSign must be fully “Finished” to count as complete).
Ideal Candidate
Licensed attorney (Illinois preferred) or highly experienced legal/compliance specialist with government procurement / provider enrollment experience
Strong familiarity with one or more of the following:
Medicaid-adjacent programs, state aging programs, human services contracting
Corporate/organizational compliance, policies, governance, and documentation
Application-based certifications and state provider onboarding
Extremely detail-oriented (because the state is not known for being chill about missing attachments)

Completed and submission-ready CCP Service Application(s) (Step 2)
A checklist of submitted items + a simple “audit trail” of what was attached and where
