We are seeking a highly organized and compassionate Care Coordinator & Intake Manager to support our residential care homes. This role is responsible for client intake, lead follow-up, care coordination, medication compliance tracking, and client retention. The ideal candidate is detail-oriented, comfortable on the phone, and able to manage multiple tasks.
This is a remote position with long-term growth potential.
Key Responsibilities
Intake & Lead Management
Answer inbound calls and messages
Call new and existing leads
Pre-screen potential clients
Schedule move-ins
Collect intake information
Follow up on pending admissions
Care Coordination
Conduct bi-weekly check-ins with residents
Verify medication compliance
Track appointments (psych, PCP, specialists)
Coordinate pharmacy deliveries
Flag residents who appear unstable
Client Retention
Identify issues early
Escalate concerns to management
Assist with service coordination
Benefits & Services
Assist with food stamps, Medicaid, insurance coordination
Track renewals and documentation
Documentation
Maintain care coordination logs
Update client records
Maintain organized files
Required Skills
Strong English communication (spoken & written)
Comfortable making and receiving phone calls
Organized and detail-oriented
Compassionate and professional
Reliable internet and quiet workspace
Healthcare or case management experience is a plus but not required.
Schedule
Part-time: 20–25 hours per week
Monday–Friday
Opportunity to grow into full-time.
Pay Range (Recommended)
Start:
$5.00 – $6.50 per hour (USD)
