We are seeking an experienced and highly organized Home Health Coordinator to support leadership and play a critical role in daily operational coordination.
Position Summary
The Home Health Coordinator provides senior-level administrative and operational support within our pediatric home health agency. This role works closely with leadership to oversee client intake workflows, staff scheduling, compliance tracking, and internal coordination. The ideal candidate brings at least one year of hands-on experience with the Georgia Pediatric Program (GAPP) and a strong understanding of pediatric home health operations.
This is a leadership-focused coordination role requiring discretion, initiative, and the ability to manage multiple priorities in a fast-paced, remote environment.
Key Responsibilities
Manage and oversee the full client intake process, including GAPP-related documentation, authorizations, and verification
Serve as a primary point of communication between families, nurses, caregivers, Virtual Assistants (VAs), and agency leadership
Coordinate and monitor nurse and caregiver schedules to ensure consistent and complete shift coverage
Track, maintain, and audit compliance documentation, certifications, supervisory visits, and personnel files
Collaborate with RNs and leadership to ensure care plans align with GAPP requirements and agency standards
Lead, supervise, and provide ongoing direction to a team of Virtual Assistants supporting intake, scheduling, and documentation
Support recruitment and staffing efforts, including applicant screening, interview scheduling, and onboarding coordination
Proactively identify and resolve staffing gaps, scheduling conflicts, and operational issues
Complete advanced administrative tasks including faxing,
Assist leadership with operational projects and process improvements as needed
Qualifications
Minimum of 1 year of hands-on experience working with the Georgia Pediatric Program (GAPP) — required
2–3 years of experience in home health coordination, administration, or healthcare operations
Strong understanding of pediatric home health compliance and staffing requirements
Demonstrated leadership experience managing, mentoring, or overseeing administrative or virtual teams
Excellent written and verbal communication skills with a professional, family-centered approach
Strong organizational and time-management skills with the ability to multitask and meet deadlines
Proficiency with scheduling systems, cloud-based tools, and electronic documentation platforms
Ability to work independently in a fully remote environment while collaborating effectively with a team
