JOB OVERVIEW

Business that is does about $3M in revenue is seeking a book keeper to organize our finances so that we can present to banks and other financial institutions on a monthly basis. We need to show a monthly P+L and balance sheet. We will use your help to send to our accountant who will file our taxes, but we are expected to give him all the information he needs. An understanding of US tax documents and filing is very important. Understanding balance sheets and quickbooks is important.

Here's the documents I will need prepared specifically. I will provide you with all the documents needed to complete.

2022 / 2023 / 2024 Return Extensions (if any) — Proof of extension filings.
2025 P&L Year-to-Date — Current year performance from Jan 1 to present.
Assets & Depreciation
Fixed Asset Register — List of long-term assets (equipment, furniture, etc.) with purchase dates/costs.
Depreciation Schedules — How those assets are being expensed over time (book & tax, if both).
Payables & Receivables
Accounts Receivable (AR) Aging Report — Unpaid balances grouped by 0–30/31–60/61–90/90+ days.
Accounts Payable & Outstanding Debts — Vendor bills due and any loans/notes payable.
Payroll & Provider Pay
Payroll & Benefits Overview — Salaries/wages plus benefits (health, retirement, payroll taxes).
Physician Compensation & Bonus Plans — Salary/bonus/productivity formulas (no PHI needed).
Payers & Billing
List of Current Contracted Payers — All active insurance contracts.
Fee Schedules & Reimbursement Rates — Clinic charges and allowed amounts per payer.
Out-of-Network Billing Policies — How OON services are billed and collected.
2024 & 2025 RCM Reports (Monthly/Quarterly/Annual) — Billing/collections KPIs and trends.
AR by Payer — Unpaid balances broken out by each insurance company.
AR Aging Summary — High-level AR totals by aging bucket.
Detailed AR Aging (>30/60/90/120 days) — Line-level detail for older balances.
First Pass Resolution Rate — % of claims paid on first submission.
Denial Rate & Appeal Process — Denials rate and steps to correct/resubmit.
Billing Process Overview — Brief description of your end-to-end billing workflow.
Revenue by Payer Report — Collected revenue by payer/source.
Bad Debt Write-Offs & Charity Care Policy — Criteria and approvals for write-offs/charity.
Corporate
Ownership Structure & Cap Table — Owners, percentages, contributions, and any changes.

File Format & Delivery Tips
Financials & AR/AP: Please provide Excel/CSV (not PDFs) where available.
Policies/Overviews: Word or PDF is fine.
Naming: Use clear names (e.g., 2023_PnL_Final.xlsx, AR_Aging_ ---------- .xlsx).
PHI: Do not include patient names, DOBs, or other PHI. Use account/claim IDs only.
Source Consistency: If reports come from your EHR/RCM system, note the system and report name/version.
